  • Application for new passport in place of reported lost passport can be obtained by calling over personally at the Consular Section of the Embassy OR by sending a self addressed stamped envelope with a request or can be downloaded from the Embassy website www.srilanka.no

 

  • All application forms are issued free of charge.

     General Information

  • Sri Lankan passports are issued only by the Controller Immigration & Emigration, Colombo. The process of issuing of a passport approximately takes three months after submitting an application.

 

 

  • If the applicant wants to have the passport received by post, applicant should provide a self-addressed, stamped, delivery envelope to the Embassy.

 

  • Once the passport is received from the Controller -Immigration & Emigration, Colombo, applicant will be notified over the phone or by a letter.

 

  • The Embassy is not responsible for any documents lost in the post.

 

  • Any inquiry relating to the issue of passports should be made to the Consular Section of the Embassy, between  9.00 - 12.30, telephone No. 23 13 69 50.

 

  • Fee, NOK 3490- could be made in cash at the counter OR to the Embassy Bank account No.7058.05.77189 DnB NOR Bank. Please make sure to send a copy of the deposit slip along with the application forms.

 

  • Please note that submission of forged documents such as Birth Certificate, Marriage Certificate, National Identity Card and other documents or making false statements relating to the Citizenship status is an offence and no refund of fees is possible in such instances.

 


     Application Procedure

  • Duly filled following application forms should be forwarded to the Consular Section of the Embassy well in advance.

 

  1. IM 35 K Form
    All the cages from 1-20 are to be completed and the applicant's signature in the second page must be attested by a Justice of Peace, Attorney-at-Law/Solicitor or General Practitioner. Please make sure to place the official frank of the attester below his/her signature.
    One photograph should be pasted at the top of the first page of the form

 

  1. IM 35 B Form
    The form should be attested by the same attester who has attested the 1M 35 K form.

    Two photographs should be pasted within the boarders of two cages on photo signature card. The attester's signature and stamp should be placed on the verification photograph.

    The signature of the applicant should be placed well within the two cages. The attester should sign the declaration and place his stamp and date.

 

  1. Form 'D'
    Please note that all section in this form should be completed.

 

  • Duly completed application forms have to be forwarded along with the following documents.

 

  1. Original Birth Certificate with two photocopies

 

  1. Original Marriage Certificate with two photocopies, if the applicant has changed the name after marriage. If the marriage has been registered outside Sri Lanka, Marriage Certificate should be countersigned by the relevant authorities.

 

  1. Copy of the detailed page of the lost passport

 

  1. If the profession needs to be included in the passport, original professional certificate with two photocopies and a letter from the employer should be submitted. Make sure to get the counter signature from the Consular Division of the Norwegian Foreign Ministry on the professional certificate.

 

  1. Sri Lankan National Identity Card with two photocopies.

 

  1. Document relating to the present visa status of the applicant with two copies. Please provide such documents with their English translations.

 

  1. Copy of the Police Entry made in respect of the lost passport.

 

  1. If the applicant or applicant's father was born outside Sri Lanka, Citizenship Certificate issued by the Ministry of Defense in Colombo, should be produced.
