  • Minors whose names and photographs are already in father or mother's passport are permitted to apply for separate passport if required.

 
  • Under mentioned documents should be submitted in addition to application forms, which need to produce for Sri Lankan passports such as

     1) 1 M 35 K

     2) I M 35 B

 

     3) Form D 3

 
  1. A letter signed by parents, giving consent for issuance of a separate passport to the child. If the parents are divorced/separated, the original of the court decision should be attached. Such documents should be countersigned by the relevant Authorities.

 
  1. The current passports of the parents should be produced with two photocopies of each pages where the child's name and photograph were endorsed.

 
  1. If the guardian applies for a passport for a child, the guardian must produce the legal documents to prove the guardianship.

 
  1. Original Sri Lanka birth certificate with two photocopies and if the child was born outside Sri Lanka the Original Citizenship certificate with two photocopies should also be enclosed.
