       SEPARATE PASSPORTS FOR MINORS WHOSE NAME WAS ENDORSED IN THE                                     MOTHER’S/FATHER’S LOST PASSPORT

 

 
  • Under mentioned documents should be produced in addition to the dully filled application forms such as,

 

 

i.                        IM 35 K

       ii.            IM 35 B

      iii.            D 3

iv.                        Form D

 
  1. A letter singed by the parents giving consent to issue separate passport to the child.

 

2.                   If the parents were divorced or separated original copy of the court decision with a letter respect of guardianship. Such documents should be countersigned by the relevant Authorities.

 

3.                   In case where the guardian applies for a passport for a child, the guardian must produce the legal documents to prove the guardianship.

 

 

4.                   Original Birth certificate with two photocopies.

 

 

5.                   If the child was born out side Sri Lanka, original Sri Lankan Citizenship Certificate with two photocopies.

 

 

6.                   A special Declaration should be given by parents, if the child has arrived in Norway in their passport.
