mentioned documents should be produced in addition to the dully filled
application forms such as,

ii.
IM 35 B
iii.
D 3
iv.
Form D

- A letter
singed by the parents giving consent to issue separate passport to the
child.
|
|
|
2.
If the parents were divorced or
separated original copy of the court decision with a letter respect of
guardianship. Such documents should be countersigned by the relevant
Authorities.

|
3.
In case where the guardian applies for
a passport for a child, the guardian must produce the legal documents to
prove the guardianship.
